The following documents can be used for birth vertification when registering with Markham Minor Lacrosse
- Birth Certificate
- Health Card (green style showing age, not red and white)
- Passport, current or expired
- Baptismal Certificate
- Confirmation Certificate
- Doctor's Note showing physicians letterhead
- School Note from Principal showing school letterhead
